Discharge Planning

Facilitating compliance to meet timely and accurate discharge plans for managed care partners.   Providing education to our partners on managed care plans’ expectations and develops facility-specific plans for improvement.

Hospital Readmission Reduction

Our case management team assists the facilities with the care and coordination of services of at risk members.  CPAN examines facility hospital discharges and provides education to aid in the identification, evaluation, and management of the members’ changes in condition. With the goal of safely reducing the rate of avoidable hospital discharges, CPAN recommends educational opportunities for related topics such as discharge planning, performance improvement plans, and condition specific training on CHF, COPD, Pneumonia and others. 

CPAN’s Hospital Readmission Reduction Project is featured on the Ohio Department of Aging website.
